Annual Medical Forms for Summer Camp

Policy on Use of the Annual Health and Medical Record
In order to provide better care for its members and to assist them in better understanding their own physical capabilities, the Boy Scouts of America recommends that everyone who participates in a Scouting event have an annual medical evaluation by a certified and licensed health-care provider—a physician (MD or DO), nurse practitioner, or physician assistant. Providing your medical information on this four-part form will help ensure you meet the minimum standards for participation in various activities. Note that unit leaders must always protect the privacy of unit participants by protecting their medical information.

 Troop 20 Leadership Elected Positions

Senior Patrol Leader (Star Rank/ Active in Troop)
The senior patrol leader (SPL) is elected by the Scouts to represent them as the top youth leader in the troop. He runs all troop meetings, events, activities, the annual program planning conference, and the patrol leaders' council meeting. He appoints other troop youth leaders with the advice and counsel of the Scoutmaster. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.
 
Assistant Senior Patrol Leader (First Class Rank/ Active in Troop)
The assistant senior patrol leader (ASPL) is the second highest-ranking youth leader in the troop. He is appointed by the senior patrol leader with the approval of the Scoutmaster. The assistant senior patrol leader acts as the senior patrol leader in the absence of the senior patrol leader or when called upon. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.
 
Patrol Leader (First Class Rank/ Active in Troop)

The patrol leader is the elected leader of his patrol. He represents his patrol on the patrol leaders’ council and appoints the assistant patrol leader. He takes the lead in organizing patrol activities at troop sponsored events, including but not limited to skit preparation, meal preparation/ clean-up, organizing patrol tent set-up/ take down, site clean-up, and works closely with other boy leaders to provide a safe and enjoyable event. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Assistant Patrol Leader (First Class Rank/ Active in Troop)
The assistant patrol leader acts as the patrol leader in the absence of the patrol leader or when called upon. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Scribe (Tenderfoot Rank/ Active in Troop)
The scribe keeps the troop records. He records the activities of the patrol leaders’ council and keeps a record of dues, advancement, and scout attendance at troop meetings. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Historian (Tenderfoot Rank/ Active in Troop)
The historian preserves troop photographs, news stories, trophies, flags, scrapbooks, awards, and other memorabilia. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Librarian (Tenderfoot Rank/ Active in Troop)
The librarian oversees the care and use of troop books, pamphlets, magazines, audiovisuals, and merit badge counselor lists. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Chaplain Aide (Tenderfoot Rank/ Active in Troop)
The chaplain aide works with the troop chaplain to meet the religious needs of Scouts in the troop. He also works to promote the religious emblems program. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.
 

 Troop 20 Leadership Appointed Positions

 
Troop Guide (First Class Rank/ Active in Troop)
The troop guide works with new scouts to help to ensure that they have a safe and positive experience transition into boy scouting. They work closely with the respective scouts and the adult leadership toward the goal of each new scout reaching his first class rank during his first year.  He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Quartermaster (First Class Rank/ Active in Troop)
The quartermaster keeps track of troop equipment and sees that it is in good working order. He keeps records on patrol and troop equipment, makes sure equipment is in good working condition, and issues equipment and makes sure it is returned in good condition. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Instructor  (First Class Rank/ Active in Troop)
The instructor teaches Scouting skills. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Den Chief  (First Class Rank/ Active in Troop)
The den chief works with the Cub Scouts, Webelos Scouts, and den leaders in the Cub Scout pack. Helps Cub Scouts advance through Cub Scout ranks and encourages Cub Scouts to join a Boy Scout troop upon graduation. The Den Chief is an important role for many reasons as it is this boy who represents us to local Cub Scout packs. This position serves as a solid stepping stone for the Troop Guide position. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Junior Assistant Scoutmaster (Star Rank/ Active in Troop)
The junior assistant Scoutmaster (JASM) serves in the capacity of an assistant Scoutmaster except where legal age and maturity are required. He must be at least 16 years old and not yet 18. He is appointed by the Scoutmaster because of his leadership ability.  He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Order of the Arrow Representative (First Class Rank/ Active in Troop)
The Order of the Arrow Representative is a youth liaison serving between the local Order of the Arrow (OA) lodge or chapter and his troop. In his unit, he helps meet the needs of the unit and will serve as a communication and programmatic link to and from Arrowmen, adult leaders and Scouts who are not presently members of the Order. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Webmaster (Second Class Rank/ Active in Troop)
The webmaster works closely with the adult leadership in the updating and ongoing maintenance of the troop’s website. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

Bugler (Tenderfoot Rank/ Active in Troop)
The Bugler should be able to make appropriate bugle calls, as requested, at troop activities. He continually strives to provide leadership through proper action to other youth and youth leaders in the troop.

* "Active in Troop" requires participation in a minimum of two thirds of all of troop sponsored meetings, events and campouts.
